The default contact form is build-in the theme, in content-contactform.php, but you’d better don’t modify it, otherwise, you can’t upgrade the theme smoothly in the future.
I suggest you can build your own contact form with “contact form 7” plugin if you want to add more various attributes. After you created contact form with it, create a new contact page without select “contact” template, you can customize your own contact page with visual composer and put the form shortcode into the page content.

Because you chagned the grid system, so that you have to change the responsive CSS at the same time.
For example, you must change the .grid-12 width for different screen as below

You must have valid license to access this content.You must have valid license to access this content.I see, sometimes it will happened but sometime it’s fine. I think maybe it’s a small bug in NHP options framework, I’m planning to change the framework to Redux framework in the later version.
Sometimes, I change another browser to upload & insert it, it works fine, so you can also try to use the other browser.
